We are an acute-care hospital with both out-patient and in-patient services.


The Medical Records Clerk will perform a variety of clerical duties involved in maintaining patient medical records to include filing, retrieving and distributing medical records, loose notes and reports and maintaining associated logs and records in accordance with established priorities and procedures in any section of the Medical Records Department.

Responsibilities include, but are not limited to:
1.  Sort, file, and collate a variety of medical records and information such as admission slips, laboratory and pathology reports, operative notes, clinic notes, discharge summaries into patient medical records in appropriate sequence and in accordance with established procedures.
2.  File and retrieve medical records by patient history number in accordance with established color coded terminal digit filing system and predetermined priorities; research lost or missing records in accordance with established procedures.
3.  Examine patient medical records for completeness ensuring all required information is included; note any deficiencies and refer to appropriate area for follow-up.
4.  Answer telephone and take requests for medical records or information; sort requests according to predetermined priorities and provide routine information of general nature such as history numbers, admission or discharge dates and medical record locations to authorized requestors.
5.  Assist the business office in retrieving and copying medical information from patient medical records to answer authorized requests in support of patient care activities, patient account settlements and third party sponsorship; document patient records appropriately.
6.  Perform other related duties incidental to the work described herein.

1.  Knowledge of basic clerical and communication principles generally acquired through a high school education.
2.  Familiarity with procedures and policies relating to all aspects of the Medical Records system, normally acquired in 6 months as a Medical Records Clerk or an equivalent combination of relevant education and/or experience.

  2. Customer Service.
  3. Filing.
  4. Telephone protocol.
  5. Medical terminology.
  6. Office procedures.
  7. Data coding – general.
